Whether it’s the result of a car accident or a fall, TBIs can have lasting effects on a person’s physical, cognitive, and emotional well-being. TBI can affect thinking, language, judgment, and behavior. No two injuries are the same, so measuring one person’s progress against another is difficult.

Our Medicaid Health Home Program is designed for Medicaid recipients facing multiple health, mental health, or life challenges. Whether you’re managing a chronic condition, navigating recovery, or feeling lost in a maze of paperwork and appointments—we’re here to take the burden off your shoulders.
This is where whole-person care begins.
Through our Health Home Program, you’re paired with a dedicated VRD care manager—your single point of contact to help you manage medical care, behavioral health, housing, transportation, and more. We build your care plan around your life, and we stay with you every step of the way.
If you are enrolled in Medicaid (children and adults) and have:
• Two or more chronic health conditions (like diabetes, asthma, heart disease)
• A serious mental illness
• HIV/AIDS
• Or have recently been discharged from:
• A hospital
• Mental health or substance use treatment
• A correctional facility
• A rehab center or nursing home
